Contrast-Enhanced Mammography (CEM)
Contrast-enhanced mammography (CEM) is a viable alternative to contrast-enhanced breast MRI, increasing access to vascular imaging while reducing examination costs. Intravenous iodinated contrast materials are used in CEM to enhance the visualization of tumor neovascularity. After injection, imaging is performed with dual-energy digital mammography, providing a low-energy image and a recombined or iodine image that depicts enhancing lesions in the breast.

How the Investigation Works
Growing tumors need a blood supply, but the blood vessels created by the tumor are abnormal. When contrast is circulated via the blood, these abnormal blood vessels leak contrast, which accumulates in the tumor in sufficient concentration to allow X-ray imaging of its distribution.

Preparation
- A complete medical history is taken, specifically including any prior conditions such as kidney (renal) function, diabetes, high blood pressure, or any reactions to previous contrast.
- A recent blood test for renal function test, Serum Creatinine, and eGFR.
- Fasting for 4-5 hours.
During the Test
Before an imaging examination, iodinated contrast material is administered to the patient intravenously. Two minutes after contrast administration, standard bilateral craniocaudal (CC) and mediolateral oblique (MLO) imaging is performed. Let your nurse or technologist know if you have pain at your IV site or if you feel any unusual symptoms such as itchiness, swelling, dizziness, difficulty breathing, or feeling like you are going to faint.
After the Test
Drink six to eight (8-ounce) glasses of water in the 24 hours after your CEM exam. Drinking water will help remove the contrast from your body.
